Welcome to the Life Bridges Transition Program Website.

 

Our mission at LB is to is to educate young adults with disabilities to increase independence in family and community settings.  At Life Bridges we work every day to provide opportunities both on the campus and out in the community for our Team Members to learn the skills necessary to reach their individual goals. Our vision for LB is for every Team Member to have a positive experience that will transition them out into the ‘real world’.   

 

The LB website is a source of information that is designed to help keep Team Members, families and the community connected and provide information that will be useful as our Team Members navigate their way down the road towards independence.

 

Here at Life Bridges we believe that the only disability is the inability to see Ability!
